If you wish to register as a patient, please ask at Reception for a ‘New Patient Registration’ pack. Our Reception staff will ask your current address to make sure that you are living in our area. Once you have completed all the forms you will be asked to attend for a ‘New Patient Check’ with a member of our Nursing team.

In order to keep GP list sizes fair, registrations are allocated to GPs on a rota basis. However, if you would like to state a preference about which GP you would like to be registered with, this will be noted and the team will try to accommodate you where possible.
